Accidents and incidents, including “near misses,” are to be documented within 24 hours and sent to your supervisor and HR as an attachment to an email. If chemicals and/or biologicals are involved, send the completed form electronically to Steve Kucera, ϲapp’s chemical hygiene and biological safety officer (CHBO), and copy your supervisor. For academic units, this means copy your department chair and dean when you email Kucera.
